Guideline:

Curriculum mapping brings together what is taught and how the instruction is delivered with the assessment results. Blending the "what," the "how," and the outcomes enables teachers to identify the instructional areas that should be monitored. Monitoring these identified patterns permits teachers to adjust the maps and the resultant instructional practices.

This link to the Rush-Henrietta (New York) School District website explains how the district integrates curriculum mapping with its instructional process.

This link is to the School Improvement in Maryland website's content on monitoring student progress. The content addresses planning, assessing, analyzing, and leading the process. Though oriented toward Maryland teachers, the content is useful to teachers in any state.
