Guideline:

Demographic data focuses on population characteristics such as size, growth, density, distribution, and vital statistics. Process data focuses on how teachers are meeting student needs. Outcome data represents the results of what is achieved through a process or program. Demographic, process, and outcome data compile a picture of a school's student achievement. Collecting such data enables the grant seeker to tell a story about who are the students, what they have achieved, and the needs they have.

The chart provides a checklist of data elements useful for an educational institution.
